How do bad cops slip through the cracks?

Most will recognize the photo of actor Matt Damon in his fictional portrayal of Massachusetts state trooper Colin Sullivan from the motion picture “The Departed.”  Damon plays the role of a dirty cop who infiltrates the state police.

Naturally, Hollywood’s portrayal is over the top, but we know bad cops exist.  Thankfully, those numbers are low and not commonplace amongst our nation’s law enforcement agencies.  But how do we ensure these people don’t get hired elsewhere?

How does it happen?  What measures exist to prevent it from occurring?  Is it the result of incomplete background investigations?  A lack of quality programs to avert troubled applicants from securing employment?  Or is it a combination of multiple factors?
